You will have to create an account first before you can login and fill out the form. Follow these steps:

  • Go to the link above and click the button “Create New Travel Account."
  • Enter and verify your travel account information, and then click "Submit."
  • We will automatically send you an email to reset your password and login. Follow the steps in the email.
  • After logging in, click on the "Incidents" button in the navigation.
  • Click the button that says "Claimant Questionnaire" to fill out the form.
